The only true monopolies tend to be granted 100% market share by the government (i.e. your electric company).

Natural monopolies sometimes exist where economies of scale give the most social benefits (consumer and producer) when a monopoly exists. Power production, water supply etc generally fall into this category.

Then there are monopolies that exist when a product is invented that, due to its societal benefit, is granted a patent. A patent of 20 years for new drugs is allowed in Canada in order that the company can recoup its investment in research in testing. It may actually cost very little to produce the drug, so competition would lower prices to a point where new expensive research could not be done.

They own and maintain the infratructure of the Nation privately and operate outside governmental regulation through a series of daughter compnanies whose business objective are outside the "Ringfence" of the regulator. These companies then "compete" with other companies for the parent companies contracts.

It's legal but to prevent the Monopoly and Mergers commission coming down on them they negotiate a percentage that must be given to external companies. This then leads to a hell of a lot of corruption and backhanders between directors all hidden by extravagent expense accounts. It is important to impress when entertaining a director when presenting you tendor for consideration afterall.
