madness and disturbing: I got a couple:
society no one ever mentions this one, and it is really warped, with a twisted mind kind of ending.
spider baby find that one. It isn't gory, it is very twisted and sick. old movie with lon chaney jr. and sid haig.
look for the ones nobody's ever heard of and try those out. You sometimes find true gems of horror. And it depends on what you want to watch. There are people who only like one kind, like zombies or vampires, or super large creatures, or sci fi. If you open your horizons to everything, you will find things you could never have imagined being created.

I only saw it once, but thought 8 MM was pretty disturbing. The Rob Zombie movies are pretty rough too, but I have seen them several times ;)

Faces of Death encompasses all of your criteria. I only saw part of one of those movies and they were the most God awful things I have ever seen. Worse than Saw or Hostel could ever dream of being.

saw 3. theres a guy who tortures you for doing little thing like smoking. and the thing is he really doesnt torture you or kill you, you kill yourself. and you dont know if this guy is mentally ill or he really hate seeing people hurt themselves and the traps are so hard to explain but you see yourself there and depending on what you do you may live but its all up to you and how much pain you can endure
