After you choose the city try to find out what its test center code is. You can do this in a variety of ways (TOEFL home page, or online). after that email/sign up for TOEFL online and show up for the test on the testing day.

You can take the test in Binan, Baguio City, Cagayan de Oro City, Cebu City, Makati City, Manila, Pasay City, and Pasig City. The TOEFL section at www.ets.org has more information about registration.
