Two meanings, depending on situation. If someone greets you with "What's up?," he/she's asking you "How are you doing?" or "How do you do?"

To answer, you just summarize your feelings/what you are doing at the moment.

If you're looking stressed/bothered and the person asking "What's up?" seems concerned about you, he/she is asking "What's the matter?" or "What's the problem?"

To answer, you just explain your problem (i.e., what the "matter" is)

Its slang for how are you doing or what's going on. Answer with ether how you are or what you are doing. BTW all the slang and abbreviations in your question makes it hard to read.
