Accountability = the state of being liable, or answerable.

Responsibility = something which a person has to look after, do, etc

Responsibility is when a person has the duty of authority for the outcome of something in terms of if it went well or not.

Accountability is responsibility with the added requirement of having to make it good if it goes bad (as in having to pay for it, or pay for fixing it).

yes, being responsible is taking charge and making sure whatever is done. Accountability means you will be held to make sure it is done, either by others or yourself. Means your to make it happen.
