The death rate means the number of deaths occurred in the particular area during a year divided by Mid year population in that particular area which is multiplied by 1000 gives the death rate in that particular area.
In notation, Death Rate ={(No. of deaths during a year)/Mid- year population)}*1000.
This is also known as Crude Death Rate.

Death rate means The number of deaths in a year per 1,000 population.
The death rate is calculated by dividing the number of deaths in a population in a year by the midyear resident population. For census years, rates are based on unrounded census counts of the resident population, as of April 1. For the noncensus years of 1981 to 1989 and 1991, rates are based on national estimates of the resident population, as of July 1, rounded to the nearest thousand. Starting in 1992, rates are based on unrounded national population estimates. ...
This entry gives the average annual number of deaths during a year per 1,000 population at midyear; also known as crude death rate. The death rate, while only a rough indicator of the mortality situation in a country, accurately indicates the current mortality impact on population growth. ...

it is used in geography and economics. it is the number of deaths per 1000 people of the population. by looking at the death and birth rate of a country, you can see how developed a country is (using the demographic transition model). this is based on the logic that the more developed a country is, the better developed the healthcare etc is, the more available it is to the population, and so people live longer and are less likely to die young or from diseases prevalent in the developing world. similarly, birth rate is high in the developing world due to lack of birth control, education etc, and lack of healthcare leads to high infant mortality rates (number of deaths per 1000 babies before their 1st birthday)

It could relate to the average age at which people die and the change of that rate for any given reasons,
for Example
In 1987 the average woman in Zimbabwe lived to 65
In 2007 the life expectancy for a similar woman is 38.
What has happened to bring about the change I am not allowed to guess.

Since it is a RATE it is number of deaths PER something else (or something else PER number of deaths.)
The "something else" is usually number of births, total population or time (usually a year.) If it is compared to number of births, it is usually referred to as the population growth rate.
