My grandmother is a big fan of her show 'Keeping Up Appearances' and its her birthday soon so I would love to get a signed photo or something like that, but I can't find a fan mail address anywhere!

I don't know how reliable or current the addresses are, but I use fanmail.biz to get names of celebrities agents and fanmail addresses. Patricia Routledge IS listed, care of the following:

You should send a large stamped self-addressed envelope with your request.

I don't think that is too difficult.
Can I suggest, that you might write to the BBC enclosing your letter for forwarding on to her. For sure the BBC will be happy to send it on to her agent, and if you ask some pertinent questions about their programme, send you some additional information themselves.
There is a published directory of actors called "STAGE" which will list their agents and contact addresses, I bet your local librarian will know it!

If you are sending a request from outside the UK, you can go to the local post office and get an international reply coupon. They can use that to cover the postage to return a letter to you.
