Cannibal Holocaust a film released in Italy and was the plot inspiration behind Blair Witch as it was made to look like a film found by people in the amazon about some student filmmakers who meet a grisly demise.
It was so realistic in it's presentation of female mutilation that Italian police almost arrested the film makers. The film shows real animals being cut to shreds, simulated vaginal penetration with bladed weapons and more. Kinda hard to find. Rated X for full nudity and lots of graphic violence it is banned in 14 countries from Europe to Asia.
Netflix has it for rent.

I'd say The Hills Have Eyes was up there, Texas Chainsaw Massacre the Beginnning, and the SAW series. Only Chainsaw Massacre is worth watching though.
Hostel definitely not, good movie but way short on gore.

The Cube trilogy are all pretty gory. So gory that five minutes into Hypercube my boyfriend couldn't watch anymore

The movie "BLOOD SUCKING FREAKS". It's about kidnapping women, keep them in the dungeon and one by one put them in the show by torturing them, like squeezing their brains out in front of an audience. The audience doesn't know it is real. It sucks.
