This has been happening since December of 2006. He usually has the same sort of appearance (same clothes) and his age is probably in the mid-twenties to thirties. He's either wearing a black or red shirt, however, she never quite sees his whole face. What could this mean? Also, he is never threatening, just hanging/milling around. When she catches a glimpse of him, she always feels like someone's been watching her.

Forgive me for seeming stupid but what do yu mean, she sees glimpses of him? In the street, in her dreams, as a spirit?

Visual hallucinations are one of the symptoms of schizophrenia. Perhaps your girlfriend is in the beginning stages of this disease.

Get her to see a doctor. The earlier her medical treatment begins, the better her outcome is likely to be.

Could be a stalker, some guy who probably likes her.
If its like only she can see him then it could be a ghost, someone from her past....
The again it could all just be in her head, imagination.. our minds a very powerful thing and somtimes our thoughts seem like a reality

Does it happen when she wakes up or goes to bed ? If so, it might be Sleep Paralysis or old hag syndrome. Suffers often feel like someone's sitting on their chest, or there is a presence in the room.
