The current theory - which is backed up by research carried out in the Netherlands - goes that, during the early stages of pregnancy the foetus is affected by a hormone imbalance which leads the brain to develop as one sex whilst the body develops as the diametrically opposing sex. For example, female brain in a male body, or male brain in a female body.

Now, as for what causes the hormone imbalance no one's quite sure, but it is known that many intersex conditions result from hormone imbalances caused by certain medications prescribed to the mother during her pregnancy, it's highly likely to be the same with transsexed people. In fact, highly probable.

As you might expect, this situation is incredibly painful for the TS people themselves, the emotional and psychological trauma of having the wrong body is immense, hence the need to transition (to live as ones true gender), hormone therapy, and surgery.
As a side note for those interested in such matters, surgeries that transsexed women commonly seek are: Genitoplasty (bilateral orchidectomy/removal of testes, penectomy/removal of penile tissue, vaginoplasty/construction of vagina, labiaplasty/construction of labia, and clitoroplasty/creation of a clitoris), breast augmentation, and Facial Feminisation Surgery.
The men have even more complex surgeries that they go through.

I must admit there is alot of info on the subject but no-one trully knows. There is like a previous post discus info in regards to hormons effecting the fetus while the mother is pregnet. Truthfully we are born transgendered and no way in hell would anyone want to choose to walk the path we have too but once you are born into the wrong body it is so hard on you that with the option of changing sex we do so that way we can be free with our inner self and be happy with life. If I never transitioned I would of commeted suacide because I was miserible being in the wrong gender, to me it was a lie and it hurt. My earlest moments as a young child was praying to god to fix me so I could wake up and feel right, I was only 5 years old and to this day I remember praying for years and years then relized nothing was going to change. There are intersex people out there and supposally the numbers are like 1 in 1000 humans born. To me I just think being transsexual is like being born with a deformation that we can fix by using modern medician and surgery. If your child was born with out the ability to walk wouldnt you seek the option to correct it with modern medical in hope so your child could live a normal life, well that is about the same with a transsexual like myself, we all just want to live and feel normal with our self plain and simple.

Actually there are now studies that indicate being transsexual is caused by the brain physically developing as one gender while the body develops as the opposite gender during the development of the fetus.

Basically they feel they are a male/female trapped in the body of the opposite sex. It's usually something they feel from a very young age, but they might not address the issue until well into adulthood.
