To be polite, you should keep your underwear on until you are invited to remove them... and then you should only remove them if you feel comfortable doing so.

The therapist usually flicks a towel over the body parts they are not working on so that you stay warm and comfortable.

Depends on whats going to be massaged. LOL
Seriously most massage therapists require you to wear something. Weather it be shorts,underware,towel, or a bathing suit. Up to the therapist. I'm sure they will inform you.

Speaking as a therapist, the patient is fully disrobed if treating lower back/leg/hip issues. For anything else, underwear is optional.

it's a matter of personal preference.

your therapist will keep that region covered at ALL times with the sheet.
