when it gets warm makes nose when driving or idle. it inline 6 with 4speed andsounds like vaccum leak. back of motor?4speed shifts and dont seem like any clutch problems.

If you think it may be a vacuum leak, move the truck to a well ventilated area and away from any buildings. If equipped with a tach, have someone sit in the cab & watch the tach.

Spray a LITTLE starting fluid where the intake manifold bolts up to the head. If there is noticable movement in the tach, then you need to tighten the manifold or replace the intake gasket.

Hard to say without hearing it.
...throw out bearing on clutch?
....bad bearing on transmission input shaft?
