Do you mean:
I want a custom picture as the background of a Word Document.
Not a problem - here is what you do:
Format
Background
Fill effects
Click the pictures Tab
Browse for the picture you want in the background.
Click ok.
You are done - the picture will remain in the background and text can go over the picture.
I hope this is what you wanted.
Note a water mark will work in the same way however the background remain white so is easy to type over. Simply choice printed water mark and the picture you want to use.

Do the following
Insert-->Picture-->from file
Then
Rightclick on the picture-->show picture toolbar -->click on the dog icon-->select behind text
If you want to appear in different style--select water mark from the 2nd icon on the toolbar
Note : if you want the same pic on all pages you must put it using header and footer

From the main menu select
Format >> Background >> Printed Watermark
select Picture Watermark and then Select Picture to choose an image.
You may want to clear the Washout checkbox if you don't want your image to be modified

It's called a watermark. Different versions of Word have the feature in different places, but usually in the Page Layout section. Search Word's help for "watermark" and you'll find what you need.

go to the format menu
go to background, then printed watermark.
click the circle for a picture, then click the button for "select picture" or something like that.
you should be able to insert a picture from there.

check to see if there is a setting - to allow a picture as a "background"
once that is added you should be able to add text to it which will appear on top - similar to how it is applied in websites
