I mean, most things in general. Procedures and policies etc that are designed to have a certain effect. Even when things are easy to adhere to, and for good reason, people always want to, and are frequently allowed to, circumvent a system.

Do you mean from a directional stand point?For instance everything that comes in a box with directions doesn't work the way its supposed to or do you mean things in life in general don't always go as plan and or work out the way you THOUGHT they were supposed to?
