Freedom, open markets, and personal ownership of property. Without these three things, residents of the third world will continue to stagnate. A select few (those who have the freedom, access to markets, and property) will benefit while the others will continue living on the dregs.
A revolution is not needed, nor are massive taxes on the rich, nor are massive government programs for the poor. What is needed is personal and economic freedom. Stroke of a pen, law of the land.

More freedom- economic and political.
The original American settlements were almost dying into extinction until each person was given his own plot of land and had to work it in order to eat. Once they got rid of the communal/socialism and everyone became reliant on themselves and then on the strength of their community, that's when they thrived.
