I am recently came to usa and i have the insurance applied by the employer. do we have to carry the any insurace card to the doctor.

Drivers Licence or ID (That's to verify its your isurance)
A list of medicines you take (Even over the counter and Vitamins)
A list of anything food and drug you are allergic too
A complete medical history of you and if possible your parents.
