Yes it's possible. I have heard of, but not seen for myself, that the polarity can become reversed.

For this to happen, it must be in series with at least one other battery. If one battery runs down before the other battery in the circuit, the remaining "good" battery can pump charge into the bad one in the "wrong" direction, eventually causing the reversal in polarity.

If the circuit isn't secure it may "Burn" out the main vulnerable aspects. curiously your television distant has a protection Zener in the circuit to stop opposite biasing the Circuit. The Zener will shunt the opposite present day to floor discharging the battery.

No. I guess the problem is .... you ;)

I suspect you had the probes reversed. You put the black to the minus and red to the plus side of the battery.

No; that should not be physically possible... they are not symmetrical internally, so I do not know how it could get reversed in manufacturing.
