If you don't want to go with the obvious choice of alcohol you could do a CD, movie tickets, a set of shot or wine glasses, gift card to a spa or a handbag.

She's turning 21, take her clubbing and have a couple of drinks, get crazy and enjoy cause she is finally legal age of drinking. If you want something more private then take her out to dinner order a cake, sing her happy birthday in the restaurant on front of everyone , buy her flowers and a card and give her a gift card to her favorite place, that way she'll shop for whatever she wants. I'm pretty sure she'll appreciate everything. Ask her what are her plans for her birthday... Maybe she wont have any. Or maybe you can throw her a quick 21st birthday party, invite all her friends and get her favorite drinks and just get hammered.

Get her something like a baby book, learning toy,
all depends how old is the baby if he/she is too small get the baby book or something soft they can play with. If he/she is one and up then get something they can get on or have more objects so he/she can explore.
