Nope... just a ticket if you get caught driving with an expired inspection.
As a matter of fact.. I get 13 months out of my inspections by always waiting until the first week of the month following the expiration month.
For example, my current sticker is dated 5/07. This means that it is 'good' through 5/31. Therefore I will wait until 6/4 or 6/5 to get my new inspection. Cops won't stop you for being a few days over due and if they do you'll just get a warning. But the kicker is.. that when you get the new sticker it will be good until 6/30/08 and then I'll wait until 7/4/08 to get it done again.
This method get's me an additional month every year on my inspections.
It always cracks me up to see people getting the cars inspected on the first of the month that their inspect expires.
Hope this helps!

It depends on where you live. In NC once you're 30 days late you'll get a bill in the mail for $200 that has to be paid before you can get the inspection, renew tags, etc. At first it was the counties with the major cities, but I think its state wide now.

No, but you'll need an inspection to obtain a current license tag.

Not that I know about - here in Virginia... Check with DMV, they know for sure - assuming you are in US :)
