That happens to everyone.
There will always be factors that we cannot control so when you make a plan, you should always have a plan B and maybe even a plan C in case the first one doesn't work.
Secondly, make sure the goals are realistic. Also turn one big goal into 2 or 3 smaller goals that are easier to conquer

let me answer with a down to earth example, since I too felt so in
my younger age. Suppose if you want to jump a wall, determine
what could be the height of the wall, at the maximum.Never target
beyond your capacity.Have the determination and will power to
do it.Keep on trying..and trying.The last one which most of people
like us never think of is, TIME-FRAME. This is the most important of
all.The moment you sow a seed, you can not eat the fruit.It has to
grow.But we do not calculate the time frame or have enough patience.Rewind your past failures-having decided on a target,
on your way to target..everything would have been OK. The idea
of nearing the target itself would have made you IMPATIENT.That's
it..that's it..Ultimately your interest would have faded out.Think of it
over,again.

during the initial stages of my professional career I was introduced to a method called PDCA cycle theory. it goes like this......P- plan, D-do,C-correct & A-act.Each stage(to be performed in that order and repeated till the goal is achieved) requires a dedicated effort to perform and a specific time frame to rise above your faculties and motivation level but at the same time to remain alert and re-course oneself towards the goal that had been set right in the beginning.Remember those sayings about failures & successes, king Bruce & the spider....failures are the pillars of success....well...better done then said.Sooooo...what are you waiting for...get cracking..and keep checking and reassuring yourself as to how many centimeters you have moved ahead compared to the last try...and eventually kilometers will take care of themselves.Do remember one thing that you are the best critic and judge of your own actions.What if you don't succeed always...there is always some fresh ideas and new skills added to your booty.....and soon enough you will get closer to whiff of success and get rid of failure syndrome.

What you need is self discipline. Find out what you're good at and what are your likes and dislikes. Work on the likes; this way you'll enjoy upgrading your skills to attain your goal.

Your plan should have the following:
It must be-Strategic
- Measurable
- Achiavable
- Reasonable/Realistic
- Time bond.
That is a SMART planing. Find out where do you fail.
