when you say how do you love him? , does it mean how much do you love him?

To me "how do you love him?" sounds old-fashioned, not something a native speaker of English would be likely to say today. If I meant "how much do you love him" I would say it that way, or, if I wanted to be formal "how great is your love for him". If I wanted to use "how" in a more literal sense of "what is the nature of your love for him" I might say "In what way do you love him?"

Usually, the question means 'why' do you love him (amlified, it means, he is not lovable very much, so how do you.). In certain contexts, it may also mean, how do you express the love for him: like when you say, how do you make coffee! Very unlikely, how much, that is, the measure of love.
