Simplified answer to match the question:
Because we all have a "mean streak" and some hidden urges to kill, abuse and generally triumph over others. The most reptilian way to survive is "kill all others".
Normally, we control those and let it out only in "hate flashes", shouting, maybe slapping, sometimes we turn onto ourselves.
But some people, more than you know, don't.
And it has nothing to do with self esteem - mass murderers usually have a very high self esteem ;-)

Well my friend it all started in a garden many moons ago.
A Loving Father put two of His children in the garden and told them they could eat of everything growing in the garden, except for the fruit on this one tree called the tree of "Knowledge" of good and evil.Unfortunately there was this cunning snake, and he was so jealous of the relationship between the Loving Father and His children that he planned to spoil things by getting the children to dissobey their Loving Father.
You see this snake wanted to have the same Love and admiration from the children that the Loving Father had.
Ever since then this cunning snake has been playing havoc with peoples minds, wills and emotions trying to get them to bow down to him instead of the Loving Father.

I'd say, most of the time, it's because of jealousy. It's a very strong emotion that leads to hatred; which is worse.
When we're drowning, we wanna pull others down, make sure they're drowning aswell. It's like ''If I cant survive, why should he?''
Most of our minds would work that way. It's selfishness. What you cant have, you wouldnt want anyone else to have it either.

That is a very good question. To elevate ones self esteem could be one reason. Another reason could be to gain power in some way.

Because in a way they want someone to be like someone they're not, and it's frustration, through self-deprivation.
