This is very serious.

Gently but firmly suggest to your friend that she speak with someone professionally qualified to help both her and her brother.

I'm afraid this is something too big to be handled by just "advice from my best friend." And remember: She trusted you enough to come to you with this, so keep her trust and don't tell the whole town. I know you won't. ;)

Did she want to have incest with her brother or was she forced to do so? I think the best thing to do is take her to a professional...she needs to talk, but with a professional who knows what he/she is doing. Geez, if only people thought twice before doing certain things....

That's very serious and wrong.
I cannot give you exactly what to do. Hope you can give additional details like is the brother older, was it consensus, how was their feelings for each other stuff like that. But I think the initial thing to do is inform their mother about it. Well, it would be hard but it would protect the both of them.

I would talk to your parents about it and then go to the authorities. Your best friend is asking for help but is too afraid to go to the authorities herself. It is unwanted sexual abuse and if nothing is done about it now, nothing will get done in the future. Sexual abuse can be mentally and emotionally damaging and it will take time for her to heal. But by being her best friend you can help her with that.
