basically it comes down to 2 things on the fast break...
number 1:
u dont have the confidence in ur ability to make the lay up. u over think it, cramp up and throw up a lay up that rolls out or hits the backboard or rim awkwardly.
number 2:
ur focusing too much on the 'possibility' of getting blocked.
with both issues it comes down to confidence in my mind. i remember when i was playing basketball in year 8 and i felt the same way. in one game i missed so many lay ups cause i was worried about getting blocked. in the one game i think i got near 10 fast break lay ups and probably hit half of them.
u need to remember that IF a player is trailing you and chasing u for the block. for them to catch up to u, jump and get their hand up to the ball and HIT ALL BALL..the odds are slim. only way it would happen is if it was a big long armed quick guy chasing u.
next time your in the fast break focus on nothing but scoring the 2 points. realistically, there is a slim chance of the defender getting you. if they reach you before u jump, take it hard at them. referees tend to favour the agressive player in that situation. try it out in one game, and i assure you you will see the benifits....

practice taking layups with your eyes closed on fast breaks. If you can sense the floor with your eyes closed while imagining the pressure, you can do it in games when you have full sight of the floor. trust me i've had the same problem. Don't worry about getting blocked, just know the floor and what's around you and you will gain confidence.

dont worry a bout the guys behind you, because heres what you have to do:
when your on your fast break down the court, make sure you stiff up your boddy, i know that sounds weird, but the stiffer you are the more contact you can take from the defender. But you see, you should just try to always remember that the defender will not usually try to block your shot, because he wants to avoide getting into foul trouble.
In highschool i had a great coach who taught me that, if your on a fast break, go up to the hoop stiff and strong, and dont do a finger-role, because fingerroles you can block very easy without having to foul.
i hope ive helped you out, but just remember basketball's not your job, it's a fun activity you do, so make sure to always have fun!!

if you are alone and aren't confident on the move try taking a 2 foot stop right in front of the hoop and take a slight pause before taking the close shot just kissing it off the backboard
if you are alone you have time to make sure you get it right
eventually though you shouldn't think while you are playing just react and let your body take over
when you are dribbling down the court just focus on the corner of the backboard and don't focus on anything else

if the defender is close to you, don't try to run faster than him, just don't let him get in front of you, just go next to him and throw your body into him or before jumping get him out of balance by puching him just a little with your arm, but not too hard and don't extend your arm all the way or they gonna call the foul on you, but you have to create contact in the air, don't let the ball go until you have the space to let it go, try a dobble pump, jump with the ball up like if shooting, get it down, get contact, and then shoot it. if you are by yourself in the layup, just jump and put your back or your leg into the defender, try to open your legs in the air, less risk of you getting down.
