Dodge mans right except I can think that no lube on the outer bearing could cause it to heat up and fail, but when you state fail wheel bolt or nut there are more than just these. You never use a air impact wrench on a wheel nut if you do you screw up the nut and stud unless you been doing it as long as Dodge man but even then you could screw up and stretch the stud. more info is required to really help, to know for sure you had to be there and be the one who did the job to know for sure. But I have caught guys forgetting to lube The outer bearing, and the race or cup being loose in the bore of the hub, and the spinning of the race can tighten the nut, by overheating and over coming the locking device allowing the nut to spin and strip. Sheesh felt like I was trying to write a book on this one. You did not state if this was after some repair or just happened, if it just happened did you ever grease the front wheel bearing once in a while? it's called to be done in the service manual at least every 30k miles or less or when a brake jobe is done it should be preformed.

Failure of any bolt is due to the "stretch" capability being exceeded.

Unseen stress risers can be a real bummer and are often associated with connecting rod failure....

I would replace all of your wheel bolts nuts with new ones.

Improper mounting torque.Too loose or too tight will damage nut/bolt threads, also wheels,rotors/drums.

i own a shop, and either it wasn't tight enough,or the stud might have stretched out on it from heat,or it could have been over torqued at some time or another, that's the most popular causes for one to fail, other than that ,it could be a weak one from the factory, who knows these days, anything can happen to them, good luck i hope this helps.

I've had this problem and it was due to the thing being overtightened, probably by an air tool.
