If your birthday is Dec 31 1990 then your birthday is Dec 31 2007 01/01/08 is just the day after your birthday.

What day are you filling in the form? If you are filling it out on December 31 you are 17 if on Jan1 you are 18. If it is to your advantage to wait until Jan1 do it. Most forms don't require one to be done immediately. You can usually wait a day or two if necessary.
