Since the field expanded to 64 teams in 1985, it has happened only 4 times. First time in 1991 when Syracuse lost to Richmond. Second time happened in 1993 when Arizona lost to Santa Clara. Third time it happened in 1997 when South Carolina lost to Coppin State. The last time it happened was in 2001 when, as someone already mentioned, Iowa State lost to Hampton.

EDIT: They all lost in the second round.

It has happened 4 times.

Most recently the Hampton Pirates defeated the Iowa State Cyclones in 2001. They then lost in the second round.

Tool is right, but I thought I'd add this tidbit: The Santa Clara team that beat Arizona had Steve Nash.
