He believes if he were to end up in an assisted living facility (nursing home), the state could come back on me for any services rendered not covered by his insurance up to the fair market value of the property for five years. Is this true, semi-true? Please help!

Your granfather has the right to sell his property to anyone he chooses and at any price. If the intent of this sale is for you to hold the property and defraud medicare - you have a problem. If the intent is for you actually buy the property for yourself as a primary residence - it would be hard to prove. What if your grandfather were to sell his home to another party for less than market value - could the government come back to the unrelated owner and sue them for his medical coverage? I do not think so.

I don't know about the state but medicare can if he goes into longterm care within 7 years of selling the home and property to you. Be sure he has a good longterm care insurance policy for this eventuality.

If your grandfather is trying to transfer his property to avoid denial of medicaid benefits, he needs to get have a lawyer prepare for him a special needs trust.. See a probate/guardianship lawyer in your state.

There is an old saying "you get what you pay for." This is too important an issue for you and your grandfather to just rely on advice given in this setting. Hire a lawyer. Pay him a few hundred dollars. It will be money well-spent.

I don't think they can come back after you .after buying his property you'll have your insurance ask the people of habitatforhumanity.com those people can give you advise or find you the right person to talk with.
