Okay, my friend has really bad breath. She's also really mean, so I'm afraid to tell her. What should I do?

Every time you pop a breath mint, offer her one. Don't say anything because you will be the butt of her jokes. If she is mean, she is insecure and insecure people attack others.
The others are right about the health issue though. Bad breath stems from many things including a stomach ulcer. She might be ill.
You can tell her about your "neighbor" or "family member" who had really bad breath and went to the doctor about it. This lil white lie isn't a bad thing, just mention how they found out she was actually sick and is now recovering and her breath isn't bad any more.
You didn't mention if this was a tooth brushing issue. If it is, then point out a few things about your own teeth like what you don't like about them and how much of a pain it is to brush three times a day and use floss.
If this girl isn't smart enough to get any of these hints, perhaps you should set her free as a friend, she will only hold you back in life.

Tell her you'll be doing the world and her a favour. You'll be the hero who did something about 'that horrible woman with the bad breath' and it may be caused by an infection that she is not aware of so she can get it checked out, plus your nose will forever be greatfull

I would offer her a mint and say jokingly, when was the last time you went to the dentist, you may want to see one. Your breath is horrible!

When someone passes by both of you while your talking, whisper in her ear "D**N that girls breath stinks like a pig" And both of you will laugh and she will be more conscious of her own breath later on! lol
