(What is your biggest failure or least successful job...) Such questions might be asked on some job interviews, the actual reason is to see how the interviewee will respond and not the answer itself.
Of this happened on an interview I'd recommend remaining confident and positive, then say something like what csucdartgirl has suggested, and be prepared to expand about it in case you were asked, but only focus on the positive lessons, how you dealt with the situation and what did you learn from it...
