My guess is that the discharge is not just blood, but contains LOTS of white blood cells (pus). This is likely a sign of infection , probably an STD like syphilis or something nasty. Tell your friend to see a doctor immediately or he will wish he had.

Ant discharge of blood, either in semen or urine, should be treated very seriously. Advise your friend to see his doctor as soon as possible.

probably a ruptured vein, thats about the only thing i know can cause that, might tell him to get it checked out
