If it is not done carefully, it can cause some medical problems. The key is plenty of lubrication. If you do not use enough lubrication you can cause tears in the lining of her rectum. Feces can get in these tears and cause an infection. If the infection is not taken care of, a worst case scenario is that it could abscess through the rectum and into the vagina. Then she would have an even worse medical problem to deal with.

Only if he (or she) goes from you butt to you coochie. With out cleaning off the Felice first. Other wise have yourself a good time. Practice on yourself a few times first. Try an enema first.

Use a product that helps to relax your sphincter muscles. Also, use lots of lube!

use lube, it shouldn't hurt, if it does take it slower, don't use a numb agent. Pain is a good indicator to take things slower or to even stop
