Why is Sandusky, OH at the top of the state and Upper Sandusky, OH is closer to the middle?

Elevation. Sandusky is on Lake Erie whereas Upper Sandusky is inland and higher up.

a lot of wearly migration followed waterways, and the St. Lawrence/Great Lakes were a major route. Thus "upper" would be measured away from the water route, which is also uphill in elevation.
