It shouldn't hurt at all to slide it back. Mine doesn't.
What happens is that perhaps as a child he wasn't told to slide it back to wash the head (which isn't a big issue since it only takes five seconds to slide it back and rub the head; even feels good). This is especially common if he's in America and his dad was cut, which is a good possibility since there is now a rapidly growing number of uncut guys. (more parents are choosing not to cut; but then again, we're the only advanced nation that does this surgery). That was the case with me, but tell him that he should slide it back each time he showers and it should eventually loosen up. It took me a few weeks, but the result is great since it's now all pleasure. =) If he says he's done that and it doesn't work, tell him to ask for this:

As for sliding it back during sex and masturbation, he does not have to to slide it back, it's all a personal preference. And keeping it back doesn't really make it better, that would be as if he were cut. The foreskin is meant to slide up and down over the head so it stimulates it since it's moist inside, hence that's why you don't need lubes for handjobs/masturbation. (think of your clit and clit hood; they're the same parts)

As for the aiming, this is the first time I hear of this, so that means that he is probably one of the few unlucky ones to have a tight foreskin. Tell him to do the things I told you above for that. Yes, if he could slide it back that's what he should do; I do it; it's not even something you think about, it just happens when you grab it.
As for cleanliness, if he washes under it (which again, takes like five seconds) that shouldn't be a problem at all. I've never had that problem; nothing grows there unless you go like a week without showering. However, if he says that it hurts to slide it back to wash, tell him to do it gradually and it'll slowly stretch out and become less sensitive. That was the case with me and now it's all perfect. =) And yes, if you take it slowly with him handjobs and sex can also help it loosen up. All of this shouldn't be happening; it's just that he's one of the few that get a tight one, usually because their parents didn't tell them to slide it back to wash when you shower.

He may not be able to retract the foreskin. Stretching exercises can usually fix that - he'll have to do them daily for several months to get the skin stretched out sufficiently. Or, in severe cases, a surgeon can make a small incision on the side of the foreskin to allow it to retract naturally. In either case, circumcision is not necessary.
In most men, the foreskin moves back and forth during sex, providing a natural lubricating motion. In a condom, that motioin still should happen. This provides additional stimulation to both partners.

He probably has phimosis. This condition does NOT require circumcision, and he should RUN from any doctor who suggests cutting him in any but the most minor way. He probably needs stretching exercises and possibly some hormone cream Sex will be better for both of you if he gets the phimosis treated. It will be worse for both of you if he gets circumcised!

He should see about non-surgical treatment without delay. Ideally the foreskin should pull back smoothly all by itself whenever he has an erection. If he can't pull it back enough to keep it clean, he could get some very nasty infections. Let him start with stretching and see a doctor for hormone cream if there is no improvement in a couple of weeks.

I agree with the other posters. It sounds like phimosis. The stretching exercises might work but it may be hard if his phimosis was caused by balantitis. He should get that checked out. If he doesn't have insurance some places have low cost men's clinics. Some Planned Parenthood's also have a special day for men. It's OK not to want to go down for sanitary reasons but don't make him feel bad... he probably already feels like crap.

I propose, the glans (penis head) is quite gentle, so is the in the foreskin. you pays interest to those components if it is no longer too extreme for him. you are able to pull the foreskin all the previously, and then it is functionally like he's circumcised (in basic terms extra gentle). or you are able to play with the floor itself, which could be countless relaxing and pleasant. in basic terms make certain he's sparkling down there, haha. it is no longer demanding and maximum men are, yet once you ever doubt, do it for him mutually as he's in the bathe. That way, it is certain. And cleanup's extra undemanding too.

I'm uncircumcised and I don't have a problem with pulling back my foreskin or going to the bathroom. I think your friend may have a non-retracting foreskin. He should see his doctor for advice about a possible circumcision.

LOL...there is a good chance of cheese. Not to mention it really makes the penis ugly with flaps of skin hanging off. He may have a problem he says it hurst to pull the skin back, it may be red and irritated or infected with cheese. Which may explain why he sits down to pee like a girl. Tell him to see a doctor and then take him to the deli and have them slice all that nasty skin off...good luck
