Get these things corrected or you might face problems. Why asking for problems when they can get avoided?

Go back to the embassy that issued the visa.remember,it could be difficult to get inside an embassy if you have no appointment card or note.If it becomes difficult to get inside, make sure you are around the embassy around the less busy period like getting to break period in the afternoon then contact the security guards at the gate or entrance. They will now be willing to listen to you if they didn't listen to you in the early hours.
Don't leave any document given to you by the embassy in addition to your passport at home.Take them all along with you.Any document,it could be in an envelope, has the same mistake in your visa.
Your passport will be taken from you together with any other documents and then you will be given an appointment ticket to back for it.This may take a few days.
Don't get frustrated with the security guards if they don't listen to you.Their job tells them to inspect appointment ticket before allowing anyone inside the building and in your case you don't have one.So by patient in asking them to allow you entry.Goo luck.

I'd call the Kuwaiti consulate in Washington, DC to make sure before you go...
