Please advise me to create a portfolio not the electronic one but on the paper. Thank you.

But really, doctors should have been constructing this portfolio throughout their careers' not doing a mad retrospective before Post Grad work (you can always rely on a nurse to point out doctors' little inefficiencies!).

A Portfolio is a collection of evidence ..... whether it is computerised or not ..... therefore your portfolio should have examples of the work you've done that addresses the different aspects of your training to date - including tutor's comments and records or your tutorials with your tutors ... the best presentation is simple and clear sections with your your best examples - you may want to add small notes to link up some of the work .... you may also want to include evidence of a diary-like notes of your experiences and thoughts relating to practical experiences your have undertaken in placements ..... the whole thing should flow and look pleasing to the eye - don't forget to also include illustrations/photoraphs with notations relating to them, if you can.....
