It was thought that he suffered from neurofibromatosis but modern research has put his disability down to Proteus Syndrome. His head was overlarge and very heavy and on his last day he insisted on sleeping lying down like other people. It is thought that his head fell backwards and dislocated his neck. In the event he died without a struggle because the cover on his bed was undisturbed.

Joseph, unable to sleep horizontally due to the weight of his head, may have tried to do so in this instance in an attempt to imitate normal behaviour.
