In the first /second season boxed set, they had an episode when in the future, a class is looking at an episode and asked that same exact question. In the current season, you can see that people do hear what he is saying, but dismiss it as a child being silly. If a kid said," I am going to destroy you" and pointed a ray gun, you wouldn't pay him much mind either. They have even gone so far as to show his detailed writings and schemes as being cartoon scribbles on paper when viewed by others.

Whether or not Stewie can be understood depends entirely on the joke being presented. There are many occations where Stewie is completely understood and carries on a conversation with other characters. While at other times, if it is funnier for Stewie's threats and rants to be chalked up as baby-talk then that is the case.

The only person who really has no conversations with or seems to understand Stewie at any time is Lois. Good thing too, considering the threats he throws her way from time to time...

Good question! This has been the subject of debate for years.

I think, that most times the "family" doesn't understand Stewie and "dismisses" what he says as baby-babble. Makes you kind of wonder if "real" babies are saying the same things as Stewie when they babble at their parents.

Sometimes they can hear Stewie, but other times they either dont hear him, or are ignoring him.

he is mostly dismissed as just babbling. when he speaks, or mostly threatens, nobody really looks at him or engage him in conversation. he is talking but nobody is paying attention to him.
except brian of course.
