When you have found another job, and have accepted the offer. Also, best to do it on a Friday or a Monday if you work a standard M-F schedule.

I would write my notice in a type of form letter when you are most happy with your place of employment. You need to leave a good impression with them in case some day you decide that you want to come back, possibly in a different department(???). When you decide to leave and have gotten a viable offer, take the letter out and fill in gaps (i.e. dates, supervisor names, etc...) about 2.5-3 weeks before you leave. Give it to them when you feel comfortable with the wording, and are ready to say this WILL be my final 2 weeks.

You can write it at any time however, you present it to your superior 2 weeks before you plan to leave.
With your addition to your question I'd say pack your desk and leave. Tell them you'd like to stay two weeks but your new employer (and don't name them) wants you to start right away.
No one should give you any problem at all. After all, if you're fired - do they give you two weeks notice? Nope.
Its all business. You leaving is a business decision not a personal one. Its like you voided a contract effective today.

extra appropriate to jot down a three week word this form you could gets a commission to take off until now. be careful to no longer burn a bridge and save working such as you the place purely employed until days end. this would enable an a good ref while a history is carried out for a clean activity.

Depends on if you are giving notice that you are quitting or are giving someone else notice they have 2 weeks to get out. It is a very uncomfortable situation either way but the sooner the better regardless.

I don't know why you should give an employer two weeks notice. A person is seldom given two week's notice before they're fired.
