I signed a contract for a month, meanign it's a temp job.
I've only worked for less than a week, so I don't know if it's possible to do so
I'm working at a job through an agent, so I'm not sure if I want to leave work early (meaning to take an hour or two's leave) who do I inform?

Also, What is an appropriate way to do this?

I thought most Temps are paid by the hour, your pay will be docked, they probably won't miss you unless its a very busy office, just ask, what harm could it do

The temp agency is your employer. It is up to them to notify your site manager. Also, any questions regarding your assignment always go through your temp agency contact and never your assignment manager or boss. This is the nature of temp work.

contact your agent at the temp agency, and inform that you must leave early on a certain time, for an appointment elsware.
aft wards, be there every day on time, and set up any plans, to happen AFTER WORK

You always tell the person from whom you receive the paycheck. In other words, tell the temp agency ... it's their job to tell the employer. Unless, of course, there's a clause in you employment agreement that YOU should tell the employer. But, start with the temp agency.
