You should go to the division where he works and file a complaint with his supervisor, the Sargent with whom he works under. So that if he has done some thing wrong they would be able to take action against him, if they find merit with your complaint. It is always better to file a complaint if you feel there has been a crime committed against you.
If you do this be prepared to be able to show he has in fact done some thing wrong to you, if you can then by all means you should.
If you are not able to show that he has done something wrong to you, then I would suggest don't file a complaint as it will only serve to embarrass you.
You can contact a lawyer to see what other options are available to you. Other than that, you can not convict an officer. He must be taken to trial first the same as any one else would be and then he must be found guilty of committing a crime, before he can be convicted of any thing.
He has the same rights as you do.
It's called get arrested, have a lawyer,
Stand trial if they decided to press charges,
Be found innocent or guilty,
If guilty get sentenced, to what ever fits the crime.
If innocent, be set free.
Same rights that every one else has.

In a court of law, you would have to prove that the officer lied under oath. Lying alone is not perjury, it has to be under oath. Just prove it.

First of all, it's perjury. Second, it isn't within your power to "convict" anyone, unless you're on a jury. You can ACCUSE someone of perjury.

First you need evidence of perjury to give to an attorney to give to the DA... then they would hold a trial, and a jury or judge would find him guilty of perjury !! Perjury is false testimony under oath.
If he/she falsified a report, you do the same things, and the charge would be "submitting a false report".
