Yes, the above pals are right that the expression "tossing and turning" denotes a state when one cannot fall asleep, or feel uncomfortable in the bed, hence comes the too figural description..Of course, we can "toss a coin", to "toss the pancakes", etc.

Tossing and turning means to go to bed and spend a sleepless restless night moving from side to side because of worry or upset, maybe.
