People use this phrase when they have had a restless night.
It means that they couldn't sleep so they spent the night moving around in the bed to try and get comfortable.

Yes, the above pals are right that the expression "tossing and turning" denotes a state when one cannot fall asleep, or feel uncomfortable in the bed, hence comes the too figural description..Of course, we can "toss a coin", to "toss the pancakes", etc.

Tossing and turning means to go to bed and spend a sleepless restless night moving from side to side because of worry or upset, maybe.
