Many never do. For small companies, health insurance is difficult to afford. The best thing to do is for the organization to join some sort of Professional Employees Organization (PEO) so they can get more affordable group rates.

If they do offer health insurance, the start date depends upon the company. Some offer it the day your start work. But, 30 days and 90 days are the most common for small companies.
