It could be stress. Or fatigue. Or maybe your mind is elsewhere. There are also some diseases/conditions that cause this to happen. In certain diseases (such as AIDS when you have a very high viral load), you have trouble concentrating on tasks, easily forget things even if you write it down, hearing someone but not HEARING them, etc. This is also some of the symptoms of dementia. (Don't worry, though. I seriously doubt you have dementia.)
Here's what I think, and take from it what you will.
I think you are young, probably in high school, maybe college. And I think you have a lot on your plate right now. These are stressful times in a person's life, and lack of sleep can compound that. It happen's to the best of us.
Just make sure you get enough sleep at night, get the proper nutrients you need, and I think you'll be fine.

very well could be ADD of ADHDor even dyslexia. Don't freak out. ask your concealer at school for help in getting a referral for some special tests to confirm your problem. you can be re trained and learn how to overcome this problem. it will be okay hang in there.

sounds a lot like ADD, ADHD includes hyperactivity and this doesn't appear to be the case with you, so you need to be prescribed dexampetamine, also known as dexedrin.
avoid ritlalin at all costs.

practice makes perfect!talk to yourself as long as u can pay attention to whatever you are saying and write down all the things that you remember since your childhood.do this always whenever you are alone.

i'm constantly honest. this has have been given me into hassle on greater suitable than one social amassing, yet i purely think of "dont ask in case you dont choose the authentic answer" no time for lies or gentle soaping :S

Usually when someone talks to you, you begin thinking not so much about what is said, but your reply..this divides your attention...We are not multi-tasking at 100%.
