The joint MSW/MPH is becoming a very popular degree especially in the policy advocacy area of any major health and Mental Health area (this can include infections - HIV, TB, etc; preventable - Diabetes, Heart Disease, etc.; degenerative - dialysis; progressive - MS, traumatic - TBI, paralysis; mental health - Schizophrenia, Bipoloar)
Obviously -work can also include direct service on the various units in hospital and other settings.

You can choose one path over the other and work exclusively in either social work or public health. Or you can find a job at the intersection of those fields such as advocacy for patients rights. It's impossible for me to give you ALL possible careers, but I can suggest a few.
HIV/AIDS Advocacy - you work on behalf of HIV patients to shape the policies that affect them, such as distribution of lower-cost HIV drugs. Or you can get counseling certification and work as an HIV counselor.
Children's Health Advocacy - work to help kids who have been harmed by drugs, abuse, disease, etc.
Geriatric Advocacy - work to help the elderly get health care, protect them from elder abuse, deal with hospice and palliative care issues, etc.
