She is a 20 year old Down-Syndrome girl who does not wear cosmetics or perfumes. She gets very sad when she finds her clothes stained. It is always the same stain color and around the neck area. It happens only to her cotton t-shirts and not her other clothes. Have tested the various detergents/cleaners in the house but have been unable to reproduce this problem. Stains cannot be removed....

If she's a teenager, it could be products with benzoyl peroxide in them.

My guess is that it's blush or lipstick or some such thing, in which case I guess you should use bleach. Advise your daughter to wash the makeup off her face before dressing or undressing; that should prevent future stains.

It might also be some sort of food stain, particularly if she's fond of popsicles, or of red sodas/fruit punch. Or, if she's in high school, it might be spills from a chemistry lab. The chem lab would explain why they wouldn't wash out. Is she in an art class? It could be paint or clay.

Is it make-up?spray the heck out of them with spray and wash wash them in the hottest setting they can take with tide and a color safe liquid bleach.(for colors)For whites, tide, liquid bleach and color safe bleach after being doused with spray and wash should do the trick(do be careful on the amount of liquid bleach you use)and it does matter what kind of detergent you use Tide is the best and whisk probably second best.

How old is she..> is it possible it could be make-up? If so try a degreaser of some sort (Gresed lightning, Dawn, Ammonia, pine-sol etc.) If it is not make-up then still try the ammonia it is good at stain removal!

Hope this helps! :)

Its possible that different parts of the tee shirt were made with different material, I would suggest the you try to do them separate and see what happens, If you still have trouble then contact the manufacturer.
