They're certified and licensed in psychology. Some have an MD in psychiatry. They counsel people with difficulties in their intimate relationships with their significant other. What they earn depends on several factors, and that varies from state to state, city to city, and even the agency within which one works. Some are in private practice - and there's too wide a range of income statistics to give you a precise figure. They'll make as little as 35 thousand a year up to five times that amount. To learn what is required to become certified in your state, you'll have to talk to your local college or university - you will need, at the very least, a four-year degree in psychology, plus a minimum of two years of intense internship.
In case you were wondering but too embarrassed to ask - they do not engage in sexual relations with their patients/clients. That could lead to decertification and even criminal prosecution. Their job is to teach and counsel, not prostitute themselves.

This profession is normally reserved for middle aged, incredibly attractive blond women, who make enough money that they don't think about it, but then get caught up in a murder scandal by sleeping with one of their more endowed clients. All fingers will normally point to the therapist, partly because of the taboo nature of her work, and partly because she's trying to cover up the sex video of her and the client (which is always recorded by the true killer), but in the end, in a near death experience, her lover/client, who as long ago given up on her as the killer, will realize her innocence and save her life by killing the killer. I think a lot of states schools offer a degree in this.
