The cervix can tear if not dilated enough. ( had one stitch myself on my cervix with first son)
You can have trilateral tearing on the outside.
Do kegels. At least 60 a day. The stronger your muscles the easier it is to push the baby out. It lowers the risk of tearing to.
massage is great keep it up.

I don't think that inside tears are very common, so I really wouldn't worry about that bit too much.
As far as avoiding a tear goes, as you mentioned massage is great. Also stay very hydrated because it helps with elasticity, and when pushing the baby out go for a controlled push. What I mean is, try not to shove the baby out too fast.

Do not push flat on your back with your feet in stirrups.
Have good nutrition in pregnancy. This will make your skin more elastic.
Do perineal massage in your final weeks of pregnancy.
Consider pushing in a squatting postion. This opens up your pelvic outlet wider than any other position.
Consider waterbirthing. The warm water helps you to stretch.
Have your caregiver use warm compresses and support your perineum during pushing.

I don't know... i'm currently worrying about this myself. I'm about to start trying massage, which is all I have ever heard... I'm going to keep checking back to see if there are any other suggestions! I"m afraid for that to happen to me!
I have also never heard of the "inside" tearing, which sounds awful. I took a child birth class this past weekend and she didn't mention anything about that when we discussed tearing and episiotomies...

My perineum tore with both of my children, but they were both very large babies. As far as tearing on the "inside" I assume you mean that her Cervix tore. That is very uncommon and only happens under specific circumstances. If you are worried about it and since it happened to your sister, you should definitely talk to your doctor about what happened with your sister.

tearing on the inside sounds serious the perineum usually dosent tear until delivery I do not know for sure but an inside tear even during delivery sounds very painful........

i never teared and i had 3 babies. all i know is massage although we never did that when i was pregnant.

yes it is possible... I tore on the inside as well as the outside myself...... I never did the the messages so I couldn't tell you if they would have worked
