Most problems with male reproductive organs are treated by a urologist. A proctologist is concerned with the rectum and sigmoid colon.

Women have much more complex reproductive systems and usually get a checkup with a gynecologist every year. Obviously more often if they get pregnant. Guys generally don't see a urologist unless there is a problem. But older guys (over 50) should get a regular prostate exam, which is not pleasant but needs to be done anyway. Much of the time, however, these are done by your regular doctor, who will send you to a urologist if s/he thinks there is a need for something to be investigated more thoroughly.

The word "proctologist" is seldom used anymore except in telling jokes. The fact is that in men, there are 2 organs which are susceptible to cancer: rectal colon and prostate. Now days, the "proctologist" is either ones Urology (for prostate/penile/scrotal/kidney/bladder issues) or Gastroenterologist for rectal issues. To answer your questions yes, starting around 50, these exams are necessary.
