Yes, report. I believe you can report anonymously as the above person said.
Think of it this way...by the slightest change in "fate", you could have been eating at that very restaurant instead of working there. Your boss/manager/supervisor should be thinking the same way also but might be a little too preoccupied with the good ol' root of all evil.
However, I'm glad that you chose to work there, because you are concerned with the health of others. I don't even want to begin to imagine how many food services I've ordered from have employees that couldn't care less...

It's definately a health food violation, considering the fact that it is an eating establishment.
However, you should keep in mind that if (or when) you report, you could be out of a job at least temporarily, if they don't raise their standards, or negative publicity the places that have violated health codes get result in a loss of business. That shouldn't stop you from reporting, just something to keep in mind.

Ewe, do you want these roaches to come home with you? They will if you don't get outa there! Start looking for a new job because this place may get closed down if they don't clean up their act. Yes it is. For the sake of everyone eating there you need to contact the health department. They need to have bug night at least once a month, like most all other restaurants do.

Yea that's definitely a violation because they're not doing enough to prevent it. My advice would be to talk to the manager personally, then contact whoever inspects your store. If you care about keeping your job you'll keep yourself anonymous when speaking to the health inspectors.

It is up to you, but you have to decide your morals. The health of all of the people you all serve or your own life and job. You mar keep in mind that they will be allowed time to fix the problem if you do turn them in.
